    Any signature (including any electronic symbol or process attached to, or associated with, a contract or other record and adopted by a Person with the intent to sign, authenticate or accept such contract or record) hereto or to any other certificate, agreement or document related to this transaction, and any contract formation or record-keeping through electronic means shall have the same legal validity and enforceability as a manually executed signature or use of a paper-based recordkeeping system to the fullest extent permitted by applicable law, including the Federal Electronic Signatures in Global and National Commerce Act, the New York State Electronic Signatures and Records Act, or any similar state law based on the Uniform Electronic Transactions Act, and the parties hereby waive any objection to the contrary.

    Submit both the new customer form and initial supply order form through the portal.

    A comprehensive email will be sent to the account email detailing website portal, portal ID, and portal password. The email will also list how to re-order supplies when needed.

    Allow 3 – 4 working days for supplies to arrive from the lab. If materials are ordered on a Friday, then materials will be processed on the next business day, Monday. Materials will be shipped via ground transportation so allow 3 – 4 days for transport.

    When supplies arrive, take an inventory of what you received. Your inventory will include the following:

    1. Requisition order forms
    2. Collection materials
      1. Specimen collection devices
      2. Tubes
      3. Cups
      4. Swabs
    3. Biohazard plastic bags
    4. Shipping materials
      1. Boxes
      2. Clinical paks (reinforced plastic bags designed for clinical specimens)
      3. Adhesive shipping labels

    Complete the following video training for sending specimens to the laboratory.

    1. Video - Processing a specimen from a patient.
    2. Video – Inputting patient information into the laboratory computer system.

    Follow the instructions below for the requisition order form.

    1. Name, date of birth, and patient signatures are the most important identifiers for each requisition order. This information must be included and match the identification information on the capture tools (swabs, tubes, or cups) and requisition order forms.
    2. Ensure each requisition order has the ordering physician's signature.
    3. Collect the specimen from the patient using the appropriate tube or cup.
    4. Package the specimen in a biohazard or specimen bag.
    5. Place the filled-in requisition order form in the dry slot of the biohazard specimen bag.
    6. Place the specimen in the specimen slot of the specimen bag.
    7. Place the package inside the plastic clinical pak.
    8. When the plastic clinical pak is full, place the plastic clinical pak in the shipping box.
    9. Seal the package by following the directions printed on the shipping box.
    10. Peel and place the shipping label on the outside of the shipping box. A sample copy of the label is available below for your review.
    11. Position the shipping box in the designated area for the shipping company to pick up from the office. If the office misses the daily pick, please take the package to the nearest shipping office, if possible.
